استخدم الحقول التالية لتوثيق التحقيق في الشكوى ومتابعة حالته.

## مرجع الحقول

| الحقل | الوصف |
|---|---|
| الحالة (Status) | تحدد مرحلة التحقيق: مسودة، قيد التنفيذ، أو مكتمل. |
| الشكوى (Complaint) (مطلوب) | تحدد الشكوى المرتبطة بالتحقيق. |
| المحقق (Investigator) (مطلوب) | يحدد الشخص المسؤول عن إجراء التحقيق. |
| تاريخ البدء (Start Date) | يحدد تاريخ بدء التحقيق. |
| تاريخ الإكمال (Completion Date) | يحدد تاريخ إكمال التحقيق. |
| منهج تحديد السبب الجذري (Root Cause Method) | يحدد المنهج المستخدم لتحليل السبب الجذري: خمسة لماذا، مخطط عظم السمكة، إيشيكاوا، تحليل شجرة الأعطال، أو أخرى. |
| الاستنتاج (Conclusion) | يحدد نتيجة التحقيق: تأكيد الشكوى، عدم تأكيد الشكوى، أو غير حاسم. |
| نطاق التحقيق (Investigation Scope) | يصف الحدود والموضوعات التي يشملها التحقيق. |
| المنهجية (Methodology) | توثق المنهجية المتبعة لإجراء التحقيق. |
| النتائج (Findings) | تسجل النتائج التي توصل إليها التحقيق. |
| السبب الجذري (Root Cause) | يحدد السبب الجذري الذي تم التوصل إليه. |
| الدُفعات ذات الصلة (Related Lots) | تحدد الدُفعات المرتبطة بالشكوى أو التحقيق. |
| التوصيات (Recommendations) | تسجل الإجراءات أو التحسينات الموصى بها. |
| الشركة (Company) (مطلوب) | تحدد الشركة المرتبطة بالتحقيق. |
